We ask that all of our new patients please take the time to download and fill out our new patient forms. As soon as you arrive, simply hand your forms to our front desk associate, and we’ll get you back into one of our treatment rooms as soon as possible! Our team is happy to file insurance claims on your behalf so you can receive a reimbursement from your dental insurance company. However, we are not in-network with any insurance plans and are a fee-for-service dental office. This allows our team to focus the most energy on the quality of your dental care.
